Consent & Release Form for Church Activities – Minor Child

I, the undersigned parent or legal guardian of the child listed above, give permission for my child to participate in activities and events held by Calvary Apostolic Church, located at 2551 W. Schrock Rd., Westerville, OH. This includes transportation on the church bus to and from events such as Vacation Bible School (VBS), Boys & Girls Club, Youth Services, Sunday Worship Services, and all other services or special events connected to Calvary Apostolic Church, from the date of this consent forward with no predetermined end date.

I understand this may include participation in church services, children’s or youth activities, and related programs. I confirm that my child is physically able to participate in these activities. Any medical conditions, allergies, medications, or activity restrictions are listed above.

If I cannot be reached in an emergency, I authorize Calvary Apostolic Church staff or designated volunteers (including Rith Yin, Michelle Yin, Jesse Brake, and/or Kelsey Brake) to make emergency medical decisions for my child.

I acknowledge and accept the risks associated with these church activities and hereby release and hold harmless Calvary Apostolic Church, its staff, volunteers, and representatives from any and all claims, liabilities, or demands arising from my child’s participation, including transportation, medical care, or any injuries sustained, including death.

I also understand and agree that: My child may be photographed or recorded during these events. These photos/videos may be used by Calvary Apostolic Church for ministry-related promotional or celebratory purposes in print or online formats, including social media.

This release is intended to be as broad and inclusive as permitted by Ohio law. If any part is held invalid, the remaining sections shall remain in effect. I confirm that I have read this agreement, understand its content, and sign it freely and voluntarily, with full knowledge that it remains in effect from the date signed forward.
