Consent & Release Agreement
I give permission for my child to participate in Fall Kids Rally at Calvary Apostolic Church. I certify that my child is able to participate in the activities except as noted above.
I understand that participation involves certain risks and voluntarily assume those risks on behalf of my child. I release Calvary Apostolic Church, its staff, agents, and volunteers from liability for claims arising from my child's participation, to the fullest extent permitted by Ohio law.
In the event of an emergency, if I cannot be reached, I authorize church leadership or designated adult sponsors to obtain necessary medical care and make emergency medical decisions for my child.
I acknowledge that I have read and understand this agreement and voluntarily consent to its terms.
